Using Google Forms, you will create a form for your classmates. Once they have taken the form, you will analyze the results.
1. Create a Google Form on an approved topic in your CET folder.
2. Your form must include the following:
- An appropriate title and description
- A variety of types of questions and a minimum of15 questions. (2 MULTIPLE CHOICE, 2 SHORT ANSWER, 2 PARAGRAPH QUESTION, 2 LINEAR SCALE, 2 CHECKBOXES, 2 DROP DOWNS, 1 DATE QUESTION, 1 TIME QUESTION, 1 MULTIPLE CHOICE GRID.)
- All questions must be required.
- An appropriate theme.
- A customized message for the confirmation page.
- Be set to require district login and only allow 1 submission per user.
- A response spreadsheet
*You also need to determine an area of interest - you don't have to stick to this but it helps to have an idea before you get started. Here are some areas of interest you might want to pursue:
- music (this can be genre or specific artists)
- animals
- cars
- food
- video games (this could be specific games or specific game consoles (PS3, Xbox, Wii, etc.))
- sports (this could be specific sports, teams, or players)
3. After your classmates complete your form, you must analyze the results and complete the analysis questions posted on Google Classroom in a Google Doc in your CET folder.

You will be given 5 days to complete this project. Here is a way you can manage your time and what you should do each day.

- Search for websites that you want to use for this project.
- Reread & review the website
- Think about the type of questions you could use in your quiz
- Multiple choice, short answer, check box, choose from a list, or essay :)

- Begin writing questions and answers
- Grammar, punctuation, and spelling will count - so check and recheck as you go.
- Remember that besides writing the correct answers you also need write down the other optional answers

Wait a few seconds... the Flubaroo application will appear next to the word Help on your Google Forms quiz spreadsheet
To grade your quiz
- Title your quiz appropriately
- Below the title of the quiz provide some basic directions
- Provide the URL (website address)
- For example, "Using this link (http://www.nationalgeographic.com/) go to the National Geographic web page and..."
- You will need to cut and paste the link into the instruction box
- Indicate what type of quiz the students will be taking - multiple choice, true/false, etc.: For example, "Through the use of this page you will read and answer..."
- Provide the URL (website address)
- Decide how you want students to register
- Will it be by first name, last name, both names, etc.?
- They will record this information through the use of text boxes

- Decide if you want to know what period they are in
- If you do, you will want them to do this using a multiple choice question format
- Provide instructions
- If there are different web pages or sections within the site you are using provide that information: For example, "To answer questions #1-4, use the page titled..."
- There are section headers that can break up the quiz so directions can be provided (look for the Add item button at the top of the left hand side of the page)

- Start typing your quiz

- Finish creating the quiz
- To reopen an already saved quiz:
- Login to Google Docs
- Double click on your Google Form
- Find the word Form - click on it
- Select Edit form
- Pick up from where you left off

- Proofread & edit your quiz
- Read and reread your quiz - make sure there are no grammar, punctuation, or spelling errors
- If desired, change the theme of the quiz -- look for the Theme button at the top of the left hand side of the page
- choose one of the 97 themes and then "apply" it (look for the Apply button in the upper left-hand corner of the web page)
- Save your work :)
- Take the quiz
- You need to do this so you actually have a set of correct answers
- Find the word Form in the spreadsheet portion of your quiz - click on it
- Select Go to live form
- Complete the quiz just as you have written it
- If you find errors, make notes so you can back and make the changes
- Submit
- You need to do this so you actually have a set of correct answers
- Proofread & edit your quiz as needed (see step 2)
- Share the quiz with me
- Find the Share button on the right hand side of the page
- Select Email editors/viewers & send it to me at hlh[email protected]
Day 5
- Score your Google Forms Quiz - to do this follow the instructions below

To install grading program**
- Login to your Google Docs account
- Open your quiz
- Find the word Tool - click on it
- Select the Script Gallery... option
- Look to the left hand-side of the dialog box, under the heading Public choose the Education category
- Look to the center of the dialog box, scroll down one or two options
- Find the Flubaroo application & click on the Install button
- Authorize the program to install
- Close the dialog box
Wait a few seconds... the Flubaroo application will appear next to the word Help on your Google Forms quiz spreadsheet
To grade your quiz
- Find and select the Flubaroo application next to the word Help on your Google Forms quiz spreadhseet
- Click on the Grade Assignment option
- Follow the steps provided by Flubaroo
- Do not allow points for names, periods, etc.
- students will have this marked wrong if they don't use the same name as you :(
